Frequently Asked Questions

1. How can I prepare for the Initial Call I make to the office?
Put your mind at ease-this will most likely consist of leaving a message with your contact information. Samantha will return your call at her earliest convenience (between 1-2 business days). During this time she will conduct a brief phone consult where she will ask a series of basic questions to get an idea of the services you are looking for. During this conversation, you should present a brief explanation of your presenting Problems, treatment expectations, and general demographic information.
2. What is the process for setting up an Initial appointment?
During the initial phone consultation, Samantha will schedule an in-office consultation with the intended client and/or the parent(s) of the client. Consultations take place Mondays-Thursday from 11am-3pm. You will receive a confirmation email prior to the scheduled appointment with necessary paperwork attached. This paperwork must be completed and returned to the office at least 3 days prior to appointment, otherwise you risk having your appointment cancelled. 
3. What information should I bring to the first appointment?

Paperwork from the office should have been emailed, faxed, or mailed at least 3 days prior to appointment. Additionally, you will be asked to bring any information that is relevant to you or your child's current diagnosis, which includes recent evaluations, IEPs, guardianship paperwork, Triennial Reports, etc. If documentation needs to be collected from professionals, please inform samantha prior to scheduling appointment in order to allow enough time for gathering necessary documentation. This can be emailed, faxed, or mailed with paperwork from the office or can be brought in the day of the appointment.

 

4. What should I do in the case of an emergency?

Unless otherwise indicated through formal agreement, Samantha L. Maietta, LCSW does not provide crisis management and cannot guarantee immediate availability. All clients should call 9-1-1 or go to their nearest hospital if an emergency or any threat to personal safety should arise.

 

5. What is the cancellation policy?

Notice of cancellation must be provided at least 24 hours in advance of any scheduled appointments. Missed sessions will be subject to a full fee charge, except under extenuating circumstances.
